Care management
When formal case management is not available, general case management can be provided. This includes an assessment of the following individuals needs:
- Health
- Daily living activities
- Social
- Durable equipment
- Financial
Referrals
Service coordinators make referrals to service providers in your county for:
- Medicare, Medicaid and Social Security
- Disability Resource Center or Department on Aging
- Home health care agencies
- Counseling agencies
- Food Programs
- Emergency Response Systems
- Medical suppliers
Additional Services
- Advocacy
- Help tenants build informal support networks with other tenants, friends and family
- Link with other service providers to coordinate case management and keep the provider current with the progress of the individual
- Link with volunteer programs
- Continue to connect tenants with new programs and update them as programs change
